Why Pain Management During IUD Insertions Matters
For many patients, the idea of an IUD insertion brings anxiety not because of the device itself, but because of the pain they’ve heard about or experienced. In reality, pain experiences are highly variable, but it is normal to be concerned about comfort during an IUD insertion. Historically, pain management during gynecological procedures has been inconsistent, often minimized, or overlooked entirely. In fact, it used to be widely believed that the cervix had minimal or no sensory innervation and therefore limited capacity to perceive pain, but this has since been shown to be incorrect, as the cervix is in fact richly innervated and capable of significant pain perception.

What Is Penthrox (“The Green Whistle”)?
Penthrox is a fast-acting, inhaled analgesic that is self-administered through a handheld device that looks like a green whistle. It has been used safely for decades in emergency and procedural settings, particularly in Australia and New Zealand, and is now increasingly recognized in Canada as an effective option for short-term pain control.

Its key advantages include rapid onset, patient-controlled dosing, and a short duration of action—making it ideal for procedures like IUD insertion that are brief but potentially painful.
The Problem With Traditional Gynecological Pain Management
Pain during procedures like IUD insertions, colposcopies, and hysteroscopies is a real and common experience. Yet many patients are still told to “just take an Advil” beforehand. This approach is unacceptable, inconsistent with current evidence and needs to change.

Is Penthrox Safe?
Penthrox has a well-established safety profile when used appropriately in low, single-dose procedural settings. It is non-opioid, short-acting, and does not require IV access or sedation monitoring.
Patients can typically resume normal activities shortly after use, although we always provide individualized guidance depending on the procedure and patient context.
As with any medication, it’s not appropriate for everyone, and screening is important. But for the majority of patients, it offers a safe and effective alternative to unmanaged procedural pain.
Who May Benefit From Penthrox During Gynecological Procedures?
One of the biggest shifts we’ve seen since introducing Penthrox is how it changes patients’ willingness to access care. When patients know their pain will be taken seriously and actively managed they are more likely to proceed with recommended procedures.
This is particularly important for:
